Lewisham use the Patient Content Store to record clinical data for COPD within a scheme they have devised known as the 'Patient Passport'. With the system, each time that a COPD patient is seen by one of the clinical team, the latest information is recorded in a consultation timeline. Additionally information is stored on diagnosis, drug treatment, and patient circumstances. The patient passport is electronically transmitted to the patient's registered GP following each consultation. The clinicians are able to request data reports from the system by any set of criteria. For example reports can be produced showing a patient's medication within different classes of drugs, together with information on severity of COPD. This information can also be downloaded from the Patient Content Store into Excel spreadsheets, where it can be used either for detailed investigation or for summarising with pivot tables and charts. The COPD system was developed in 2005. Prior to going live with the system, Lewisham used Teleologic's content management software to import data for several hundred patients from existing Excel and Word files. |
